Associations Between Comorbidities and Severe Maternal Morbidity

Objective To evaluate the associations between number of chronic conditions and maternal race ethnicity (race) with risk severe morbidity. Methods Using National Inpatient Sample, Healthcare Cost Utilization Project, Agency for Research Quality, years 2016-2017, we examined morbidity among 1,480,925 delivery hospitalizations women different races numbers comorbid using multivariable logistic regression. Results The rate was 139.7 per 10,000 deliveries. Compared no comorbidities (rate 48.5/10,000), there increased one comorbidity 238.6; odds ratio [OR] 5.0, 95% CI 4.8-5.2), two 379.9; OR 8.1, 7.8-8.5), or three more 560; 12.1, 11.5-12.7). In regressions, similar were noted (adjusted [aOR] 4.4, 4.2-4.6), (aOR 6.6, 6.3-6.9), 9.1, 8.7-9.6). Black had higher rates than all other racial ethnic groups, 55% (95% 54-56%) having comorbidities, compared 67% 67-68%) White women, 68% 67-69%) Hispanic 72% 71-73%) Asian women. Conclusion We found a dose-response relationship morbidity, highest comorbidities. Focusing on prevention treatment childbearing age may have potential to improve outcomes across ethnicities.
